Care Provider Job Description: Position Summary:Supported Living Services (SLS) consist of Personal Support Services and Training and Habilitation Services which include 24-hour Emergency Assistance.The SLS Caregiver assists the client with a variety of support services based on assessed needs. These services may include, but are not limited to:
Individual Personal Plan:The SLS Caregiver must be familiar with his/her assigned client's IPP (Individual Personal Plan) goals and ensure that services provided to the client are directly related to those goals. A copy of the most recent IPP is included in the client's file. An annual IPP meeting is held when the client celebrates his/her birthday, which is scheduled by the client's Service Coordinator. The SLS Caregiver should advise his/her supervisor of any IPP meeting to which he/she has been invited. No caregiver should attend an IPP meeting without his/her supervisor, unless directed to do so by that supervisor or the Executive Director.Paperwork Responsibilities:The SLS Caregiver reports directly to his/her assigned supervisor. Each Caregiver is responsible for completing a weekly schedule of appointments with assigned clients. This schedule should be turned in to the supervisor every Monday of each week before noon. Caregivers are also responsible for preparing progress notes, which should be submitted no later than Monday of the week after services are performed. Caregivers are also responsible for progress reports on each assigned client. These reports are due quarterly, unless otherwise requested. Once completed, the reports should be turned in to the assigned supervisor review. |
